The guiding principles (Table 1) included the broad categories of design, navigation, access, function, community, language, and content display with a clear distinction between what the website design “Must be” and what it “Won’t be” from a user perspective. The system is unique, as it addresses navigation and communication, alongside resources and possible time frames for treatment. The system is intended to give immediate access to information on services for accessing stroke care.
The system’s pathway has 8 steps to ultimately provide guidance on the best route for patients with stroke care (Figure 5).

Overcoming the Digital Divide for Older Patients With Respiratory Disease: Focus Group Study
Many experienced difficulties with navigation, particularly when opening a new tab and returning to the original page, and the website’s layout was criticized because it was considered difficult to use by patients with visual impairments. Although the potentially favorable role of monitoring was acknowledged, this was only considered relevant if it was simultaneously monitored by health care professionals as part of a treatment and follow-up plan.

It is particularly relevant that these web-based navigation resources and e-hubs have been community [19] and expert driven [18]. Although the availability and complexity of these navigation tools has increased, no single e-hub is recognized as a gold-standard model.
The COVID-19 pandemic has highlighted the importance of accessible mental health services [20] and has resulted in a surge in the demand for web-based treatment options.

To date, a very few studies have implemented navigation services within primary care, integrating these services within the breadth of care they coordinate [32-37] and supporting the PCPs’ efforts to engage patients in self-care. We found a single study that evaluated the role of a patient navigator in providing system navigation support to more complex primary care patients; however, in that study, the navigation services were provided by a social worker [38].
