The intervention had an average duration of 10 weeks and consisted of biofeedback-mediated PFMT, functional exercises, and education (Table S2 in Multimedia Appendix 1), as previously described [41]. Briefly, after enrollment, participants completed an onboarding form with demographic and clinical information and selected a PT specializing in pelvic health who oversaw and continuously tailored the treatment.

This technique of providing direct feedback on bodily signals is called biofeedback [9]. Diaphragmatic breathing has been shown to support relaxation, well-being, and anger regulation [10,11].
Previous research has shown the potential of DEEP to reduce negative emotions. First, a randomized controlled trial with DEEP in undergraduate students showed a decrease in anxiety symptoms after using DEEP 4 times [12].

Integrated biofeedback (BF) and virtual reality (VR) technologies have been widely applied in recent years [1,2]. Accumulating evidence has shown that the application of integrated BF and VR technologies may mitigate users’ stress levels [3,4], anxiety [4-6], self-reported pain [7,8], and developmental disorders [9,10], and improve people’s cognitive function [11,12].

One nonpharmacologic alternative is biofeedback, a mind-body therapy that provides sustained pain relief [13,14] in various clinical settings [15-26]. Biofeedback reduces pain by teaching patients behavioral modifications (eg, decreasing respiratory rate) to change their physiological status (eg, increasing heart rate variability [HRV]) [27], characterized as the increase and subsequent decrease in heartbeats during inhalation and exhalation, respectively [28].

These long and structured sessions, however, limit the ability of individuals to institute an HRV biofeedback program into their daily lives. Short sessions of HRV biofeedback might therefore provide a greater impact if they are able to elicit an autonomic response. Interestingly, short sessions of HRV biofeedback can successfully modify HRV and improve the regulation of emotional reactivity and therefore warrant further evaluation [45,46].

Gamification, VR, and biofeedback (BF) are promising options. However, our research indicates that few studies have simultaneously used these technologies for specific phobias. Virtual reality exposure therapy (VRET) uses 360° computer-generated simulations [8,9] similar to traditional exposure therapies [2]. Meta-analyses have shown that VRETs are effective and their performance can rival standard exposure therapies [2].

There are four candidate coaching components being delivered, with a fifth untested component that all participants will receive the following:
Daily resting HRV biofeedback: the participants will be asked to record regular HRV measurements in the morning before eating or drinking.
Momentary HRV biofeedback: throughout the workday, the participants will be asked to take in-the-moment HRV measurements, with the option of performing a mindfulness activity to manage elevated stress.

The Precious app combines MI with heart rate variability–based biofeedback to strengthen the mental link between an individual’s actions and well-being. Biofeedback has been found to reduce stress and anxiety about physical activity, removing a barrier for some inactive individuals [42]. A pilot study also found that biofeedback improved quality of life and reduced fatigue in a small sample of women with chronic fatigue syndrome [43].

This study has two phases: (1) the development of a serious game biofeedback app called the Fit Lab Game and (2) the testing of the app with participants in a pilot study.
In accordance with definition of serious games by Djaouti et al [25], the Fit Lab Game is meant for physical and mental training in the health care sector and intended for the general public.
